TUCSON, AZ (Tucson News Now) -

A letter from the U.S. Office of Special Counsel urges President Obama to take action on problems within the VA system.

It all started in April when the Phoenix VA was put in the national spotlight. People claimed patients were dying while waiting for appointments.

Now, the agency says it's investigating more than 50 cases nationwide that allege threats to patient health.

Whistleblower employees across the country have provided information for the open letter. An employee at a VA in Arkansas said, "Suction equipment was unavailable when it was needed to treat a veteran who later died."

In Texas, a VA official confirmed a whistleblower's allegations that "The facility was not paying fee-based physicians in a timely manner, resulting in some physicians refusing to care for VA patients."

The agency recommends hiring someone within the VA system to make sure these problems are being corrected and that disciplinary actions are taken when needed.
