Beginning Tuesday, parents will be asked to fill out what Metro Nashville Public Schools officials are calling one of the most important forms in the district's history.
Metro students should bring home two forms Tuesday, one of which is the household information form.
The form will ask parents about their family's income. That information will be used to determine how much federal funding the district gets.
That form used to be on the application for free and reduced lunch at school. Since Metro schools now offer free meals to all students, that is no longer an option.
The other form is a student information form, where parents can confirm all of their child's personal information is accurate.
